Registration Information
Fort Knox Parent Central Service is located in the basement of the One Stop Processing Center Bldg 1384.
CYSS Registration and sign ups Monday-Friday 0800-1600
Walk-in Registrations: Monday-Friday 0900-1200
Registration Appointments are scheduled from 1300-1500
Parent Central Services is closed for staff training 3rd Thursday afternoon of each month (1300-1630)
Late appointments available upon request
All patrons are required to attend a Program Orientation prior to using services.
The following documents are required to complete Registration:
- Completed Registration Packet for new registrations or re-registration
- Proof of Eligibility (Sponsor/Child Military ID card or Guardianship documents)
- Child's Official Immunization Record
- Two Emergency Contacts (name, address, telephone number)
- LES and/ or pay statement and recent W2 form
- Health Assessment or well baby check up (due within 30 days of registration)
- A current sports physical must be presented at the point of signing up for Youth Sports
- Current Family Care Plan for single or dual military Families
- Health Screening Tool
- Annual registration fee is waived in support of the Army's Commitment to Families through the Army Family Covenant
