Do you have an idea on how Ft Knox can improve the quality of life for the Army Family (soldiers, family members, retirees, DA Civilians, reservists, and National Guard)?  If so, complete this form and return it By clicking the submit button at the bottom of the form. It will be emailed to the AFAP Office.

 Our local conference provides an opportunity for installation leaders to meet with members of the Army Family and will be held January 24-26, 2006 at the Fort Knox Leaders’ Club.  Valid issues are forwarded to specific work groups for discussion and possible resolution.  Issues must have a measurable end result and impact the total Army Family or a particular category of the Army Family.  Personal issues are not considered valid issues.  The AFAP is not a forum to address personal complaints, but rather  to identify those issues that concern and impact the total  majority of the Army Family.  Please type of print legible the information requested.

 

 
Issue Title: Make the title as short as possible, simple, and clear to anyone not familiar with the issue. Do not give a detailed description here.
 
SCOPE: Use complete sentences and write in paragraph form. The scope must answer the following questions: What is the problem? Why is it a problem? Who does it affect?

 


 
RECOMMENDATION: This is the solution to the issue stated in the SCOPE.  Keep it simple, clear, and concise.  Be very specific in terms of what needs to be done. Use bullet comments.
